This program changes lives in the most impactful ways. Combining the expertise of an occupational therapist, with the funds to complete some home modifications and/or add assistive technology devices, North Dakota has delivered a unique and powerful tool for people who have traditionally fallen through the cracks.
Who Qualifies for this Program?
North Dakotans 60+
- Who own or rent a residence in the state and want to continue living there for as long as possible
- Have annual income that is not greater than 400% of poverty level. Currently around $64,000 for an individual.
- Need help functioning in their current home
North Dakotans 18 to 59
- Living with a physical disability that makes it difficult to function in their current home
- Who own or rent a residence in the state and want to continue living there for as long as possible
- Have annual income that is not greater than 400% of poverty level. Currently around $64,000 for an individual
*This program is not designed for individuals already enrolled on a ND Medicaid Waiver.
What Can I Expect?
- An Occupational Therapist led consultation of your needs and wants as you look ahead to living in your home
- Additional therapies as needed including physical therapy, speech therapy, or nursing (up to four visits)
- Reasonable home modifications that contribute to an individual’s ability to function in their current living environment
- Assistive technology to enhance ability and longevity
- A detailed discharge plan with a path to sustained success
- All at NO COST
How Do I Apply?

- The project manager will review the application and determine eligibility
- There may be a few follow up questions
- The project manager will inform you about your acceptance or denial
